Talking & Planning
We sit down to talk about your situation and goals, review and assess your loved one’s health, and determine their overall needs to find any gaps in their care. From there we’ll create a personalized plan of action. Once we begin, regular home, hospital, or facility visits can be scheduled to monitor and optimize your loved one’s care.
Navigation & Management
We are a project manager for your loved one’s health. We’ll connect you to the appropriate medical and non-medical resources, schedule appointments and follow up after, and make sure everyone on your health care team is working together to achieve the same goals.
Vigil Planning
Vigil support can be in private home, care facility, or hospital. Assisting in the design of a personal plan for the final days and hours of life. They provide the support and physical presence that patients and families need. The needs and desires of the dying patients and their loved ones differ so we will tailor our approach to fit the individual families unique wants and needs.
Patient Advocacy
We are your voice in the healthcare system. We can accompany you to appointments, and talk directly to the physicians, medical service providers—even insurers—to address your concerns, translate “doctor-speak,” and advocate on your behalf for the most appropriate care.
Legacy Projects
Leaving a gift of a legacy for loved ones. We will explore what your legacy is and incorporate pictures, videos, stories, music, wisdom, and experience of the dying person. There are no limitations on how one can create a legacy.
Community Education and Speaking Engagements
Topics include death and dying. End of Life care to churches, hospitals, doctors’ offices, veteran’s group.
